There are a few ways to go, but first of all, check out Named Ticks in MIDI Designer.

Then: you could either have MIDI Designer in the middle -- receiving the CC and then sending a program change to the AxeFX (supercontrol -> subcontrol) -- or you could just have MIDI Designer reacting to the program change.

Hey man, you are really good at supporting your app. I have shoocked! :)
Right now I go to upgrade Mididesigner lite for the pro version.

One more question: have any idea to introduce a list of the presets to remember the numbers? a multi line label would be used, but need 128 lines, better something else that can be scroll

There is no way to do a scrolling list yet in MIDI Designer. Named Ticks give you a knob, slider or crossfader that shows just one value at once. A scrolling list will happen in the future.

Note: if you're asking how to get the names INTO the list, check out the "Edit Text" button for Named Ticks. That way you can make the list with a text editor (or Excel -> Text Editor) and then just paste them in.
